Required forms to be turned in to Coach Igou by November 16th

       - If you participated in a fall sport you need:

          Section 7 of the physical form 

          Proof of Insurance Form

          Emergency Contact Form

          Athletic Trainer Form

          Sudden Cardiac Arrest Review Form

          Parent/Athlete Consussion Statement

 - If you DID NOT participate in a fall sport you need to complete the    entire physical packet found in the main office.

All athletes must have a phyical and concussion baseline test prior to beginning practice.
