CMS Nurse's Office
All physical forms must be signed by a parent/guardian (form can be downloaded at the bottom of this page). Parents should also download the Sports Emergency Information form (also below) and send that form in with the physical form.
Students must bring the complete forms to the nurse's office in order to
sign up/reserve their physical date in June. All physical forms for the
2012-13 school year must be dated after June 1st, 2012.

Attention Current 8th Grade Students/Future 9th Grade Students
Here are the dates for 9th Grade Sports Physicals
Monday 6/4/12 - 4:30-6:00 - Freshman Football Only (PW Nurse's Office)
Tuesday 6/12/12 - 1:30-4:30 - All Other 9th Grade Sports (PW Nurse's Office)
All interested athletes should sign up by May 25th by calling (610) 825-1500 x1932. PIAA forms can be downloaded from the CSD website under Plymouth Whitemarsh High School - Athletics - Physical Form.

Parking on Maplewood Drive (Aside the track/football field):
On October 24, 2005, Plymouth Township Council passed Ordinance No. 802 which states that there is to be No Parking on Maplewood Drive when using the field. There is no parking on either side of Maplewood Drive which also includes the top to bottom of the street.
*Please plan on parking either at the upper parking lot near the middle schoolbuilding or at the very top of Plymouth Road.Thank you for your cooperation in this matter.
