REQUEST FOR NON PUBLIC TRANSPORTATION FORMS:

Colonial School District requires proof of residency before transportation arrangements can be made. Below is a list of the required documentation. Each year district families will be required to update proof of residence information.

Proof of Residency;

o   Student’s Parent is a Homeowner 
      §  Deed; or
      §  Property Tax Bill; or
      §  Settlement Papers

                  o   Student’s Parent is a Renter (Lessee) 
                              §  Copy of Lease; and
                              §  Landlord Verification Letter; and
                              §  Colonial School District “Lessee Affidavit.”
                                   ·  Annual renewal required.

                  o   Student and Parent are living with a District Resident 
                              §  If District Resident is a Homeowner:
                                   ·  Deed, Property Tax Bill, or Settlement Papers; and
                                   ·  Colonial School District “Multiple Occupancy Form.”
                                         o   Annual renewal required.

                              §  If District Resident is a Renter (Lessee)
                                   ·  Copy of Lease and Landlord Verification Letter; and 
                                   ·  Colonial School District “Multiple Occupancy Form.” 
                                         o   Annual renewal required.
