 Transportation Bus Stop Change Request Form

 

Colonial School District requires proof of residency before transportation arrangements can be made.  If you are a home owner, please send a copy of your deed or current tax bill.  If you are leasing a property, you must fill out the "Non-Public Transport Lessee Residency Affidavit" (link to form is below).  If you are living with another family, you must fill out the "Non-Public Transport Mult Occupancy" form (link to form is below).  Please complete and notarize the residency form that pertains to your situation.  Please note that we must have an updated proof of residency submitted each school year.
